November 4, 2007 Seniors' Pill Records Now Close at Hand

Assisted living centers go digital with medicine

Charlotte Observer

by Kirsten Valle (kvalle@charlotteobserver.com)

Pineville --At The Laurels and The Haven assisted living centers, gone are the days of paper medication records, crowded charts with notes scrawled into tiny spaces.

Now, staffers distribute pills with the help of a handheld computer and digital records, a system meant to save time and curb errors.

The Laurels and The Haven at Carolina Place installed the TOCCARÉ (pronounced tow-CAR-ee; the word is Italian for "to touch") medication management program in April, becoming the first assisted living facility in the area to make the switch to this kind of system, facility spokeswoman Stacy Carter said.

"This system is virtually error proof," she said. "It reduces the error rate down to a fraction of a percent."

Assisted living centers report that as many as one in 10 long-term care residents a month have an "adverse event" as a result of a medication error, said Marc Cameron, CEO of the Oregon-based Daverci Solutions, which developed TOCCARÉ.

Since facilities began using the new system in 2003, they've said errors are down by about 80 percent, he said.

Hospitals began using computerized programs years ago, but assisted living centers have long relied on paper record"We thought, is there a way to use to make the whole communication safer. "We just thought there must be a better, safer way."

The new system uses personal digital assistants, or handheld computers, with residents' names, photos and medication staffers log in using a fingerprint scanner.

The data is stored at the assisted living center, the pharmacy and in Oregon at Daverci Solutions.

Carter said the system is a time-saver. Instead of spending 10 to 15 hours a month recording changes, Laurels and Haven staffers can update the records immediately.

And instead of faxing orders for refills to the pharmacy, staffers can order medications from their handheld units.

Medication technician Donna Byers said the new system was easy and efficient.

About 90 of the 100 residents there are medicated, and many take 10 to 12 different medications. Byers said the TOCCAR system cuts an hour or two off each of her rounds.

Quicker, safer programs are becoming more important as the assisted living industry grows, said Debi Norman, a nurse consultant with Brookcare Pharmacy Services, which distributes TOCCARE.bout 1.3 million doses of medication through the system a month's available in 20 states, up from eight last year, said.

David Kyllo, executive director of the National Center for Assisted Living, an advocacy group based in Washington, D.C., said the system catch on even more.hasn't seen research about the effectiveness of computerized systems, "but it is logical," he said. "It has the potential to help."

In coming months, The Laurels and The Haven Highland Creek in University City will move to the new system, Carter said.

And TOCCARE will evolve.

"Computers are dynamic, and every couple of months, we will have a new upgrade,"
